It seems that just one subject have the problem, I solved it by force inside skull and now I am computing the gain matrix for all of them

If you are using @christianoreilly 's templates, I guess the BEM surfaces were created using a different algorithm, probably MNE's mne_watershed. @christianoreilly Is this correct?
